Please have all necessary forms filled out through the portal at least 24 hours before your appointment.

Insurance Information

While Table2Mat does not accept insurance, you will receive a super bill that you can submit to your insurance company for reimbursement.  I recommend calling to check with your insurance company beforehand so you know their reimbursement policy for medical nutrition therapy.

Cancellation Policy

If you cancel less than 24 hours before your appointment, you will be charged $50.  Cancellations made prior to 24 hours, as well as rescheduled appointments will not be charged a fee.

Late Policy

Please respect my time and your own time.  If you are late to your appointment, you will not be given the extra time at the end.
